SCULPTING K — LIABILITY WAIVER & ASSUMPTION OF RISK By submitting this application and engaging in any training program with Sculpting K, I, the undersigned, acknowledge and agree to the following: 1. Voluntary Participation: I voluntarily choose to participate in physical fitness activities and understand that they carry inherent risks of injury. 2. Medical Clearance: I confirm I am physically able to participate and have consulted a physician where appropriate. 3. Assumption of Risk: I assume all risks associated with my participation, including but not limited to muscle strains, sprains, fractures, and cardiovascular events. 4. Release: I release Sculpting with K and Keishlanis from any liability arising from my participation in the program. 5.
